Kadlec Health System and its Affiliates (collectively "Kadlec") are pleased to offer My K-Chart - a secure web-based application that lets you access certain health record information including: active medications, problems, allergies, some laboratory results , immunizations records, health reminders, appointment history discharge instructions, and billing information.

We reserve the right to revoke access at any time for any reason.

Electronic Communication – If you think you may have an emergency situation do not use My K-Chart for communication. Contact your physician’s office or call 911.

My K-Chart gives you a secure, convenient way to send and receive messages from your physician’s office. Kadlec will work hard to give a timely response through My K-Chart. There may be times though when it takes up to three business days to respond to you. It is important that you not use this system if you need urgent attention.

Responses to requests are based on the information you give. If there is not enough detailed information given we will not be able to provide you accurate or reliable service.

If you send a message through My K-Chart, it will be shared with staff that assists in providing your service. If you need to discuss sensitive information, you may want to do so only with your provider (i.e., by phone or face-to-face) and not through My K-Chart.

E-mail Privacy – You will be notified through e-mail when there is new information to be viewed on My K-Chart. This means that anyone with access to your e-mail, such as your spouse or employer, will be able to see the notification from My K-Chart. Although no private health information will be sent in the email, you should take this into account when providing an e-mail address.

You are responsible for protecting your password.

Your password is not known by anyone at Kadlec. It is only known by you and can be changed online by you at any time. Your login name and password are key to protecting the confidentiality of your information. Please DO NOT SHARE your password with anyone.

User names and passwords provide two layers of authentication and are stored in an encrypted database that is isolated from the Internet.

Proxy Access - If you are age 18 or older you may ask for proxy access to another person’s My K-Chart account. Proxy access may be given if you are the patient’s parent or legal guardian (see Minors below), if the patient signs an agreement to allow you access, or if you are the patient's healthcare power of attorney. To get proxy access you must complete the Proxy Access portion of the My K-Chart Consent Form from you Kadlec Clinic. You will be asked to provide proof of your right to access health information of the patient. Only one person will be given proxy access to a My K-Chart account. If your legal relationship with the patient changes or the patient revokes authorization you must inform Kadlec immediately by contacting your Kadlec Clinic office or sending written notice to Kadlec Health System, attention My K-Chart, 888 Swift Blvd, Richland, WA 99352.

Kadlec reserves the right to revoke proxy access at any time for any reason.

Minors – Due to Washington State and federal laws, access to your child’s My K-Chart varies based on the child's age and the type of health care information involved. You can request proxy access to My K-Chart accounts for your child under the age of 12 years. Because of the complexity of the laws dealing with access to children's records, your access to your child’s My K-Chart will automatically be removed when your child reaches age 12. However, you can ask for proxy access to a My K-Chart account, with limited services, for your child between the ages of 12 and 17 (inclusive). All access to your child’s My K-Chart will be removed automatically at age 18.

Minor children between the ages of 13 and 17 (inclusive) may request their own My K-Chart account with limited services.

Security and Confidentiality – Kadlec uses administrative, physical and technical safeguards to protect all confidential information, including the information accessed through My K-Chart. Kadlec is committed to protecting the confidentiality of your health information. We limit employees’ access and ability to enter or view information based on their job. Firewalls, passwords, encryption, and audit trails are used to safeguard your information.

My K-Chart must be accessed with a Secure Sockets Layer (SSL) compatible browser or terminal (Netscape or Internet Explorer versions 5.0 or greater). Our SSL web server uses authentication and offers the highest level of encryption technology commercially available (128-bit RC4).

You can tell when you are secure by looking at the location (URL) field. If the URL begins with https:// (instead of http://), the document comes from a secure server. This means your data cannot be read or deciphered by unauthorized individuals. You can tell whether you are truly connected to Kadlec Health System and Affiliates by viewing the digital certificate. This certificate verifies the connection between the Kadlec Health System and Affiliates server's public key and the server's identification.

Third Party Healthcare Applications

At your election, MyChart allows you to share your health information maintained in MyChart with external third party applications (e.g., apps). While MyChart contains tools to help you manage the permissions you may grant to such third party applications to access your health information, the external third party applications may or may not have the safeguards and protections that MyChart does. We do not endorse or validate any third party application for use with MyChart.
